Case Management Discharge Coordinator Jobs in Texas

Screening and assessment skills and expertise in care coordination and/or utilization management in a health care setting required.

Minimum Education and Experience Requirements:

Five or more years of related experience may be substituted for the credentials.

BSW, bachelor's degree or LPN required.

Active and current appropriate licensure through the Department of Professional Regulation, State of Florida, as indicated.

2+ years of Case Management experience

Conducts comprehensive evaluation of Members using care management tools and information/data review

Utilizes negotiation skills to secure appropriate options

Utilizes influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health

2 years experience in behavioral health, social services.

Long Term Care experience preferred

Five (5) years of supervisory/management experience in case management or a field directly related to the specific duties of the position.

Directly monitors case management staff to ensure proper and effective job performance.

Develops, coordinates and implements training for case management personnel.

Performs other duties as assigned. Job Requirements Minimum Requirements

Case Management Coordinator (FT) 7a-7p Weekends - Johnson City, TN

JOHNSON CITY MEDICAL CENTER, Johnson City, TN

Full-time (scheduled 72 hrs or more per pay period)

7 AM to 7 PM

Ensures the design, implementation and evaluation of a cost-effective unit-based orientation process that focuses on knowledge, skills and relationships

Coordinates/Conducts required training to close performance gaps in knowledge and skills

BLS certification from the American Heart Association required

2 years clinical nursing experience required

1 year preceptor or educator experience required

Discharge planning experience highly preferred
