Has experience in care coordination or case management, preferably in a mental health or behavioral health setting

Facilitate coordination of care for psychiatric medication management and therapy services across a spectrum of treatment settings and levels of care

Strong leadership and organizational skills, with the ability to manage multiple priorities and projects simultaneously

Cross-functional management: you thrive in cross-functional environments by delineating roles and responsibilities, over-communicating, listening deeply and asking questions

Provide leadership, overall management, strategic direction, and support to the Care Coordination Team.

Develop and execute operational strategy designed to deliver on an excellent patient experience

7+ years of experience in healthcare patient experience and contact center operations, including at least 3 years in a leadership role.

Excellent communication and interpersonal skills, with the ability to build effective working relationships with internal and external stakeholders.

Strong analytical and problem-solving skills, with the ability to use data to drive decision-making and continuous improvement.
