Position Title: Aging Life Care Manager Associate

1. Minimum Education: Bachelor’s Degree, Master’s level clinician is preferred.

2. State Licensure as a Social Worker: LICSW, LSW, Registered Nurse, Physical or Occupational Therapist is required.

Bachelor of Social Work required, or equivalent combination of education and related experience.

Assists care managers with cases requiring emergency intervention; discusses, with the social worker, care plan and means to facilitate patients’ goals.

Maintains documentation in the patient chart in accordance with department requirements; prepares and submits reports as required.

Maintains documentation of case activity in the patient chart in accordance with hospital requirements.

Identifies unmet biopsychosocial needs and under the supervision of coordinator; assists patients to meet those needs effectively.
