We are currently seeking a Wellness Program Coordinator for our Connections for Living (Memory Care) Department at our community, The Courtyard at Mt. Tabor in Portland, OR!

Schedule: Full-Time, Tuesday - Saturday

At MBK, we believe that every resident has the potential to experience joy and pleasure, connection with others, purpose, and self-esteem at every point on the spectrum of the cognitive changes we call dementia. The purpose of the Wellness Program Leader in memory care is to uncover and develop each resident’s potential to have these positive daily experiences and to grow. This goal is achieved by creatively planning and implementing a daily Wellness Program using the structures and content provided by MBK.


Essential Job Duties

  • Contribute content to monthly community newsletter
  • Support community sales and marketing efforts by effectively demonstrating programs and providing materials to post on social media – 5%:
  • Assist the team to find better ways to meet residents’ needs when they express distress through verbal or nonverbal communication
  • Document Wellness Program services provided in each resident’s Service Plan
  • Highlight Wellness Programs during tours or when other guests are present
  • Optimize the use of MBK-provided content, ie. iN2L, Eversound, Joy for All Companion Pets, SingFit, etc.
  • Provide training and support to ensure that Wellness Programs are conducted when the Wellness Program Leader is not present, including evenings and days off
  • Understand each resident’s wellness needs and document them in the Service Plan – 20% :
  • Provide training and supplies to enable all memory care team members to support residents’ greatest potential – 10%:
  • Identify resident’s interests, needs, and potential roles in the first 30 days after move in and incorporate them into the neighborhood’s Wellness Program plan
  • Effectively manage program supplies and services budget
  • Take videos and photos of Wellness Programs and provide them for posting on the community’s Facebook page or other social media
  • Create and execute daily Wellness Program using the Daily Flow structure
  • Ensure that adequate and appropriate supplies are always available for team members to initiate individual, small group, and large group experiences
  • Attend required Teams calls and trainings
  • Encourage and instruct all memory care team members to build relationships with residents, communicate effectively with them, and support their abilities
  • Plan individual, small group, and large group experiences by reviewing Lifestyle Surveys and finding common needs and interests
  • Plan and implement Wellness Program Daily Flow – 60% :
  • Review the Lifestyle Survey for each new resident prior to move-in and create a plan to welcome and integrate the resident into the Wellness Program beginning on move-in day
  • Perform administrative tasks necessary to maintain role requirements – 5%:
  • Contribute to inter-community discussions and projects on Teams groups and channels
  • Ensure that every resident has large group, small group, and/or individual experiences included in their Service Plan

Additional Responsibilities

  • Understand and comply with all Federal, State, and local regulations, and all company policies and procedures concerning the department
  • Manage expenses within established monthly and annual budgets Minimize waste and misuse of supplies and equipment
  • Perform other job duties or special projects as assigned or requested by the Memory Care Director or Executive Director
  • Promote and lead with a spirit of teamwork and open communication in accordance with the MBK principles and core values
  • Communicate with caring and professionalism with residents, team members, family members, and other guests
  • Maintain a safe and secure environment for all staff, residents, and guests following established safety standards, policies, and procedures

Requirements

  • Current First Aid Certification
  • Basic knowledge of Alzheimer’s Disease and other dementias
  • Excellent communication skills, including ability to speak, write, and read English
  • Basic computer skills, including Microsoft Office Suite (Word, Excel, Outlook, Teams, etc.)
  • Negative TB test
  • Vaccination (per government regulations)
  • Willingness to work TUE-SAT or SUN-THU schedule
  • Background clearance (per government regulations)
  • Prior work experience in a similar role supporting people living with dementia or other cognitive differences
  • Ability to empathize with others, understand their needs, and respond in a helpful way
  • Ability to communicate sensitively and professionally with team members, residents, family members, and guests at all times
  • Over the age of 18
  • High school diploma or GED
  • Understanding and compliance with all regulations regarding Resident’s Rights

Physical Demands

  • Push a resident safely in a wheelchair
  • Walk up to 5 miles per shift
  • Bend, kneel, squat, sit, reach
  • Assist in moving residents in emergency situations
  • Lift 25+ pounds
  • Exert 10-25 lbs. of force occasionally and/or frequently to lift, carry, push, pull, or otherwise move objects
