This is not a remote position. Work will be performed in call center environment.

Under the direction of the Triage/On-call Supervisor this position is responsible for the patient and/or care giver needs via the telephone during regular hours, after hours, weekends and holidays.
Primary Responsibilities:
Prioritizes and responds to calls received, in a timely manner. Incorporates nursing skills and appropriate questions to accurately evaluate problems/needs and instruct the patient/care giver in accordance with the plan of care. Provides emotional support appropriately during the telephone conversations to patients/caregivers. Determines appropriate discipline to respond to call and/or visit patient/family. Conducts follow up calls as problems/needs arise or as requested by the team members.
  • Communicates/documents with staff, physicians, community resources, vendors, providing accurate information regarding patient’s needs/problems.
  • Fulfills mandatory skills training, as required.
  • All other duties as assigned.
  • Maintains and audits hard copy of patient information as needed.
  • Documents all telephone communications with patients/caregivers via computer in a timely manner per CHNE protocol.
  • Receives daily team reports and updates from primary nurses.
Required Licenses/Certifications:
  • Current State of Florida drivers license with driving record acceptable to Community Hospice insurance carrier.
  • Must currently be registered in the State of Florida as a Registered Nurse.
Qualifications:
Agency for Healthcare Administration (AHCA) Level II Background Screen
Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
  • Minimum of two (2) years of experience in medical-surgical, oncology or home health setting.
  • Possession of a degree from a diploma nursing program, 2 year Associate Degree, or Bachelor’s Degree in Nursing

  • Ability to work within framework of multi-disciplinary team.
