Triage Registered Nurse (Remote)

Care Hospice is hiring Full-Time (Remote) Compassionate Triage Hospice Registered Nurses (RNs) to join our after hours Customer Service team. If you are looking for a career with a purpose and a supportive team culture, then this is the position for you!

Our Hospice Triage Nurses serve as a clinical contact for patients and consumers of healthcare with medical questions or seeking medical advice via telephone. Hospice Triage Registered Nurses (RN)s are vital to improving patient care outcomes. Triage Registered Nurses handle pain and symptom management according to the patient's plan of care. Triage Nurses use assessment skills to assist with any resolution of calls as well as contact the On-Call RN to make visits for those patients that need a visit. The purpose of this position is to provide excellent customer service to our hospice clients

• Responds to after-hours calls from hospice patients and their families; triages calls and makes after hours visits assignments for visits when warranted.
• Hospice and/or home health experience is stronly preferred.
• Complies required documentation in a timely manner and uses the nursing process to monitor patient status.

• Reviews clinical referral information and responds to request and inquiries as appropriate. Collaborates with the Administrator on call regarding the eligibility and appropriateness of client for hospice services.

• Bachelor's degree in nursing or health care preferred but not required.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
