Summary

Supervises the daily staff activities of Social Worker staff in addition to their normal social worker responsibilities. The social worker provides patient education and emotional support, is responsible for referrals to community agencies, coordinates patient care with other team members, and assists with concrete services.

Detailed Responsibilities

• Performs staff responsibilities to fulfill required service levels. Leads the handling and resolution of complex issues and complaints.
• Provides direction, orientation, training, coaching, and mentoring to staff. Performs or assists with performance evaluations and disciplinary actions.
• Assesses quality of services delivered and facilitates staff development programs. Ensures staff compliance with departmental and organizational policies, procedures, and protocols.
• Responsible for program's patient's social needs including discharge planning for inpatient and outpatient. Responsible for evaluations to include psychosocial assessments in order to determine appropriate candidacy and continue to work with patient and family. Provides ongoing assistance with social needs to all patients. Responsible for the presentation of patients at selection committee as required by CMS where appropriate. In addition, oversees and is a resource to other specialty social workers.

Other Information

Complexity of Work: Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

Required Work Experience: Five (5) years' experience.

Active participation in social work professional organizations preferred. Certified Clinical Transplant Social Worker (CCTSW) credential preferred where applicable. LCSW preferred, MSW required.

They also have the area’s largest pediatric and neonatal transport team, which also does mobile ECMO. JDCH serves over 375,000 children per year from around the state. Approximately 100,000 children are cared for in our 3 emergency rooms across the county per year. JDCH’s services continue to grow with the opening of the new specialty center/ambulatory surgery center in Palm Beach County in 2018 and a new ambulatory pavilion in Miramar, near Miami-Dade County which will open in 2021.
