When it comes to what you’ll do as a Specialist, Person Directed Supports at UCP, here’s what to expect


  • Establish and sustain a productive relationship with parents and guardians of Agency with Choice participants, and the designated Managing Employer and Support Service Professional(s) of each participant.
  • Meet in person with the Managing Employer to review satisfaction and compliance with the Agency with Choice services, reviewing supports provided and completion of all required annual paperwork.
  • Record all communications to and from Managing Employers, Support Service Professionals, and all other team members regarding Agency with Choice services.
  • Update daily activity logs and transportation Logs with specific participant outcomes; including the distribution of required forms to Managing Employers and Support Service Professionals.
  • Manage and audit all daily activity logs by reviewing for electronic visit verification accuracy, auditing for over utilization, and ensuring proper service documentation. Communicate with the Managing Employer if over utilization occurs and with the Support Service Professional if additional documentation is needed.
  • Provide re-education training and on-going training for participants, Managing Employers and Support Service Professionals.
  • Attend all meetings including Individual Support Plan meetings and any additional team meetings to support the participant to ensure services are delivered appropriately.
  • Develop and enhance partnerships with public and community agencies.


What we’re looking for in a Specialist, Person Directed Supports at UCP


Minimum Requirements


  • A valid driver’s license
  • Ability to read and interpret documents, ability to read and communicate verbally, and ability to comprehend written communication and translate in English or Spanish or vice versa
  • Flexible to work all assigned shifts as business needs dictate
  • Spanish/English bilingual required
  • Must be at least 18 years of age
  • Knowledge specific to the needs of individuals with intellectual and developmental disabilities, autism, and/or the aging population; developmental planning and goal setting
  • 6 months to 2 years of previous job-related experience
  • Access to regular use of a vehicle with valid insurance coverage
  • High school diploma or General Education Development (GED) certificate required


Preferred, But Not Required


  • 2 to 5 years of previous job-related experience
  • Regulatory compliance (i.e. Chapter 2380, 6100, 6400)
  • Associate’s degree or technical certification within a related area


While performing the duties of this position, a Specialist, Person Directed Supports at UCP will


  • Continuously sit, use a computer, express and exchange ideas, communicate with others, and apply visual acuity less than 20 inches
  • Rarely stand, walk, twist, move 11-50 pounds, bend, stoop, squat, kneel, climb stairs, reach above shoulders, walk on uneven surfaces, apply visual acuity greater than 20 feet, and apply visual depth perception and field of vision (if vehicle driving duties assigned)
  • Occasionally move 1-10 pounds, and apply color vision perception
