Job Description
We Are Hiring:
  • Department: Case Management
  • Schedule: Monday-Friday
  • Hospital: Ascension Sacred Heart Pensacola
  • Location: Pensacola, Florida

What You Will Do:
Support various services in case management including collaborating with the care team and educates patients or their caregivers as to the case management process.
  • Expedite discharges by transmitting appropriate documentation to providers for the acceptance of patients. Document all interactions with payers and communicates status with appropriate staff.
  • Identify and document all referrals to appropriate facilities or agencies.
  • Advocate for resources and removal of barriers.
  • Facilitate information and support to empower the patient to make ongoing independent medical and/or healthy lifestyle choices.
  • Identify and escalate quality of care issues. Provide status updates through established channels with the case management staff. Liaison between case management, admission, payers, etc.
  • Utilize negotiation skills to secure appropriate options and services necessary to meet the patient’s benefits and/or healthcare needs.
What You Will Need:
Licensure / Certification / Registration:
  • Social Worker preferred.
Education:
  • Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
  • High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
