Tryfacta is seeking a Social Workers for our clients in Batavia, NY 14020. This role is an 5 month contract assignment.
Pay Rate: $35.00/hr.
Schedule: 8 AM to 5 PM
Job Duties

• Work as and assures all department staff workas member of the NYSVHB team. Other facility staff view Social Services Department as helpful and supportive of their efforts.
• Develope and facilitatesupport, problem solving and advocacy group, including Resident and Family Council.
• Provide appropriate input regarding residents 'Care planning; attend accurately participates in meetings.
• Provide staff with resident rights, orientation, and mandatory in-service training.
• Maintain accurate and timely medical chat documentations on each resident as required by regulations and facility policy and procedure.
• Supervise and assist with pre-admission, and discharge plans for residents to include obtaining all necessary doucumentation.
• Assists with resident care functions if help is needed.
• Provide counseling and support as needed to residents and their families.
