Job Description
Responsibilities:
Creates optimal outcomes for patients/families by managing complex psychosocial and economic co-morbidities. Through advance practice skills, mobilizes internal resources to reduce risks and assists patients/families in accessing external resources.

Qualifications:
Required Education:
Minimum: Master's Degree in Social Work (MSW)

Required Experience:
Minimum: 1 year in social work or human services field, preferably in acute care setting and/or Chemical Dependency counseling.
