IU Health Ball Memorial Hospital is looking for a Social Worker to join their Case Management team. This position will work either Saturday-Monday or Friday-Sunday, 12 hour shifts each day. The Social Worker in this role will work both on a micro and macro level. You will be involved with patients and families for direct care, as well as involved in process improvement initiatives and creating standard work in an effort to effect change on a larger scale. Licensed BSW and MSW candidates will be considered for this role!

The Social Worker cares for patients who have complex psychosocial needs or require assistance from social support programs or community assistance. Interviews patients and relatives to obtain social history relevant to medical problems and planning. Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care. Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies. Interprets and assists in resolving social problems that relate to medical condition and/or hospitalization.
- Requires proficiency in Microsoft Excel and Word.
- Permanent LSW licensure preferred.
- Requires strong counseling skills to help patients and families understand, accept and follow medical recommendations.
- Requires knowledge of the principles of growth and development over the life span.
- Requires strong clinical assessment, planning and goal development skills.
- Requires knowledge of state and county adult and child protective legal mandates.
