Social Worker - Part Time

Assess needs and locate community resources for patients and families in need of concrete services such as transportation, lodging and food; Develop/maintain effective working relationships with community agencies.

Assist patients/families with issues requiring intervention within the hospital and community.

Consult and collaborate with health care providers and other staff regarding patient care, problems and discharge planning.

Counsel, plan and implement discharge and post-hospital care with patients and families considering age specific needs.

Document social services provided appropriately and in accordance with department/hospital policy and maintain department statistical records. Participate in department and hospital committees as assigned.

Interview patients and families for social information relevant to treatment and health care planning and compile comprehensive social evaluations.

Provide crisis intervention, emotional support, counsel for adjustment to illness/disability for personal/family problems related to health/illness on bio-psychosocial issues and complex situations. Refer patients and families to community, state and federal agencies and resources and provide coordinative services. Perform legal reports for suspected child abuse, elder abuse, domestic violence, and guardianship referrals.
