Social Worker Care Coordinator

Comfort Keepers is a locally owned and operated licensed home care franchise since 2004. With over 800 Comfort Keepers franchises, we are growing and are ranked in the Top 10 overall. This is a Non-medicare and non-medicaid company. Comfort Keepers is looking for a Social Worker/Care Coordinator to improve the quality of services and support for our seniors/clients. We are a home health care team operating under the Minnesota Department of Health Comprehensive Home Care License. We serve primarily seniors. Our territory includes St. Cloud area as well as Brainerd Lakes and Buffalo.

This position will work M-F 8am-430pm. No weekends, evenings or holidays.

The Coordinator will support clients, family, staff and office co-workers to help deliver quality services and support for our seniors. This is a new position joining our supportive team.

• Comfortable learning new software and electronic charting
• Social Work or Care Coordination Experience/Education
• Drivers License and Clean Driving Record
