National Health Foundation is looking for a new Social Services Associate at our Recuperative Care program in Oxnard, CA. This is a great opportunity to provide assistance and guidance to individuals that are experiencing homelessness. Our next Social Service Assistant will bring their knowledge and experience, while delivering these services to our guests in a compassionate and empathetic manner. We aren't just looking for someone to check off the boxes, we are looking for an individual that is passionate and committed to our mission while believing in the radical hospitality that is being provided to the homeless community.

Pertinent Information

Schedule for the position is Tuesday - Saturday 8:00 am-4:30 pm. This is a non-remote position. Salary for the position is $40k-$50k ($19.23-$24.03/hour). This position is a non-exempt, full time hourly position.

What does success as a Social Services Associate look like?

• Assist in completing referrals and coordinate services for mental health, housing, and other social services
• Provide support in completing needs assessment and care plans
• Complete Coordinated Entry System ("CES") Documents
• Support in completing discharge notes and disseminate to hospitals
• Maintain up-to-date database
• Support in facilitating discharge process and locating housing for individuals
• Attend care coordination meetings

Requirements

• Bachelors Degree Required, preferably in Human Services or Social Work
• Ability to communicate effectively, and bring a positive, upbeat, and professional attitude to our team
• Excellent time management and strong organizational skills, with computer proficiency in Microsoft Office Suite
• Minimum of three (1) years relevant experience in healthcare delivery, hospital education, social services, or similar field preferred
