Summary of Job Duties:
ESSENTIAL FUNCTIONS:
Assesses potential recipient and family/caregiver perception of physical and psychosocial implications of transplantation.
1. Actively participates in transplant program community Outreach efforts.
2. Attends and participates in the pre transplant education sessions and pre and/or post-transplant clinic as assigned.
3. Works effectively with medical and surgical program directors, inpatient units, outpatient clinics and department personnel to optimize patient and family centered care
4. Performs physical assessment if required
5. Follows through, in a timely manner, with orders and protocols for diagnostic studies for recipients.
6. Assesses diagnostic study results, makes appropriate referrals and coordinates care with other disciplines
7. Provides patient and family education
8. Contributes to the education and acts as the resource person regarding transplantation for all staff nurses, referring physicians and outside agencies.
9. Communicate effectively with referring physicians
10. Participate in research where appropriate.
11. Attend transplant conference and participate in the patient selection and problem solving related to patient care and the department.
12. Work efficiently and effectively with Electronic Health Records and database system
13. Take call for organ offers and after hour calls for patients
14. Triages patient problems over the phone according to established protocols, state regulations and own assessment skills.
15. Coordinates the transplant process to include organ acceptance from local OPO and import organs, compatibility verification, patient notification, assist with OR arrangements
16. Understands the United Network of Organ Sharing (UNOS) policy as it relates matching of donor and recipients and organ acceptance.
17. Provide continuum of care in the office, unit, and clinic during absences of other coordinators.
18. Provide public and professional education where required
19. Actively involved in quality assessment and process improvement activities
20. Participates in quality metrics and data collection as needed

PRE TRANSPLANT PHASE: (15%)
I. Assures the performance of necessary studies to determine a patients candidacy;
II. Participates in both patient and family education;
III. Maintains appropriate monitoring of patients’ status while on the deceased donor organ
transplant waiting list
IV. Works with the financial counselor on insurance issues and requirements;
V. Submits data for patient on UNOS waiting list;

TRANSPLANTATION PHASE (15%)
I. Participates in multidisciplinary rounds
II. Coordinates inpatient plan of care with multidisciplinary team
III. Documents patient results in the EHR
IV. Communicates with pharmacy/pharmacist for discharge medications/ billing
V. Educates patient and family post-transplant
VI. Assists patient in appropriate medication box use
VII. Prepares patient education packets for post recipients
VIII. Verifies patient home medications
IX. Coordinates transition of care between inpatient and outpatient
X. Communicates with dialysis units post-transplant
XI. Coordinates with referring physicians post-transplant
XII. Prepares patients for discharge and outpatient follow-up
XIII. Completes regulatory forms and patient forms

Qualifications:
Minimum Qualifications or Competencies (Essential for the job – the applicant must have at the time of hire) Basic competency include but are not limited to education, experience, licensed require, or physical requirements.
Education, general:
Graduate of an Accredited School of Nursing.
Experience, length in years and kind:
4 years nursing experience.

Current Arkansas State Board of Nursing License
