Requirements

• Strong technical knowledge of social work and discharge planning processes
• Bachelor's Degree in a relevant field
• Valid license and current registration issued by the New York State Board of Nursing
• Excellent leadership and team management skills
• Minimum of five (5) years of previous experience in social services and discharge planning within an acute care hospital setting
