Job Description
Clatsop Care Memory Community is seeking a Resident Care Coordinator. The Resident Care Coordinator performs duties related to resident services week and has the duty of administering all non-injectable medications to assigned residents, provides treatments as delegated, supervises assigned staff, and performs other duties as assigned below.


ESSENTIAL DUTIES AND RESPONSIBILITIES Include The Following


  • Contains routine audits according to the facility QA program.
  • Enters resident specific information into the computer as needed including Physician Orders,
  • Order supplies and DME through appropriate company per family request
  • Obtains POLST for every resident, registers with OHSU and assures they are posted in resident cupboards.
  • Audit med room with facility RN to check for expired meds and other issues.
  • Demonstrates good faith effort to support, maintain, and improve the health and quality of life for residents.
  • Works with Administration to conduct investigations in resident incidents and communicates incidents to Physician and interested parties.
  • Maintains current knowledge of medications administered and manner of administration.
  • Helps Team Leaders arrange staff coverage for call in's, ensures all shifts are covered and is available if needed. Proactively communicates potential staffing issues or concerns to the Administrator.
  • Practices basic infection control techniques.
  • Assures house medical supplies are maintained and readily available.
  • Input Physician Orders into Matrix and check MAR for accuracy.
  • Ensures appropriate use and destruction of classified drugs with the Health Services RN.
  • Facilitates to degree possible resident involvement in self-care.
  • Participates in service planning.
  • Assure meds are being ordered properly and in a timely manner.
  • Maintains confidential nature of work.
  • Assists and instructs care givers in the provision of resident care including dressing, grooming, bathing, nutritional intake, toileting, and incontinent care. Helps out on floor to assure tasks are being demonstrated properly.
  • Informed of resident's health history and current condition(s) and service plan. Makes all changes as necessary.
  • Demonstrates knowledge and interest in the provision of care for persons with Alzheimer's and other dementias and has the ability to work effectively with this population.
  • Provides and ensures appropriate administration of oral medications in accordance with service plan and Physician's orders to insure optimum of resident comfort and health as needed.
  • Performs delegated and/or assigned nursing tasks per Oregon Administrative Rules as needed.
  • Manages Resident weekly and monthly weights and vitals.
  • Documents nursing interventions and responses to care.
  • Manage staff time sheets every other week and notify Administrator of overtime. Verify payroll of all nursing staff.
  • Coordinates, develops, and posts staffing schedule "monthly and weekly" in cooperation with staff.
  • Documents any change of condition.
  • Reports resident change of condition in accordance with established policy and procedure.
  • Participate in development, knowledgeable and supportive of resident service plan goals.
  • Orders medications from resident's Pharmacy of choice with new admits as needed for TL assistance.
  • Assigns or attains resident vitals per physician orders and service plans.
  • Obtains all 90-day orders for residents.
  • Other duties as assigned by Administrator.
  • Medication administration records, and service plans.
  • Provides wound care and treatments as delegated and with return demonstration of ability as needed.
  • Supervise and assign staff on shift and participate in hiring, evaluation, and termination of care givers and team leaders.
  • Keeps medication supply areas organized and tidy.

Qualification Requirements


  • Written evidence of current CPR training is on file.
  • Performs detailed assignments, works under pressure.
  • Demonstrated ability to administer oral medications and perform treatments accurately in accordance with requirements and as delegated.
  • Preferably has successfully completed an Oregon State Board of Nursing approved training program for the administration of non-injectable medication.
  • Defined and optimal goals.
  • Reads, speaks and writes the English language.
  • Relates positively to residents, staff and visitors.
  • Has the ability to prioritize tasks and work efficiently.
  • Has strong supervisory and team leading skills with the ability to motivate staff to achieve

  • Must function independently, have flexibility and the ability to work effectively with residents, personnel and support agencies and maintain a calm environment.
  • Must be able to fill in for positions that are needed out on floor.
  • Push and pull wheelchairs and other equipment occasionally.
  • Able to ambulate, bend, stoop, kneel, squat and move a maximum of 40 pounds during routine care of an assigned Group of Residents for an 8-hour shift.
  • Frequent walking.
  • Visual and auditory activity within normal limits with or without prosthetics.
