Job Description
The Rehab Counselor will provide vocational rehabilitation guidance, counseling, work adjustment and pre-job placement services to assist program participants in personal development, preparation for competitive work, training and other employment options.

Essential Job Functions


  • Prepares and submits required reports to the Department of Rehabilitation Services, Department of Developmental Services, and other appropriate agencies.
  • Provides necessary counseling for crisis intervention and refers to appropriate social service agency if required.
  • Performs other duties as assigned.
  • Develops performance goals and behavioral criteria for participants/workers with the assistance of Employment Specialists/Production Supervisors in the form of an Individual Written Rehab Plan (IWRP) according to CARF standards.
  • Revises and or modifies participants’ rehabilitation plans and reassigns participants as determined by progress evaluations.
  • Writes discharge summaries; quarterly reviews of IEE and any other documentation required.
  • May have to attend to the personal care issues of others.
  • Represents CW Inc. at conferences, seminars, and public and private meetings relevant to program activities of the Rehabilitation Division as required or as requested.
  • Solicits and recruits prospective participants for referrals and conducts intake interviews.
  • Evaluate participant’s work performance and social adaptation on a regular continuing basis, keeps individuals informed of progress, and maintain records and any other documentation.
  • Monitors department’s statistical data with respect to goals and planning.
  • Participates in regularly scheduled team meetings.
  • Monitors and reports on participants’ progress with respect to readiness for placement; notifies Job Developer of participant’s readiness for placement and cooperates with Job Developer with preparation for placement and job-seeking skills.
  • Provides direct counseling of program participants/workers.
