Duties

VA offers a comprehensive total rewards package: VA Nurse Total Rewards
FUNCTIONS:
The primary responsibilities of the Women's Health Nurse Navigator are to track and coordinate all Mammography, Paps, Maternity care, Women's community care and Preventive care for women Veterans within the Philadelphia VA Health Care System catchment area.
  • Educates patients on available women specific services including maternity care and promotes increased utilization of gender-specific services, such as Mammograms and Pap smears in compliance with VHA clinical guidelines and standards.
  • Supports the WH program by participating in outreach activities including women's health fairs and education campaigns.
  • Functions as a liaison between the patient, contracted provider, and the local VA medical facility (providers, Fee Office, and Pharmacy).
  • Notifies providers of needed follow up care. Sends reminder letters to patients due or overdue for their annual Mammograms and Pap tests.
  • Maintains statistical data on maternity care including:
  • Monitors biopsy results and follow up.
  • Serves as WH liaison to HCS out-patient clinics
  • Monitors test results to ensure communication of test results to the patient is documented. Screens and tracks referrals and internal consults to ensure timely access to all three services across the continuum of health care.
  • Ensures the coordination of maternity care throughout the local health care system, including Community-based Outpatient Clinics and outpatient clinics that are distant from the main facility.
  • Maintains a minimum of bi-monthly (every other month) contact with the patient while she is in the care of the Fee Basis provider. Documents contact information in the electronic health record including any complications and notes the name and location of the outside Fee provider or facility.
  • Monitors and coordinates DEXA scans and serves as liaison with the Endocrinology Service
  • Completes and updates the Mammogram, Pap screening and Maternity care tracking data base for women Veterans.
  • Monitors, tracks and coordinates care for maternity care Veterans. Monitors the delivery of services and tracks maternal and fetal outcomes, including coordination of non-VA maternity care with relevant VA providers, especially if the Veteran has pre-existing medical or mental health conditions.
  • Facilitates access to mammography services, assures annual cervical screening as appropriate, and collaborates with teams to ensure timely follow-up of all related test results with appropriate action.
  • Maintains knowledge of local and community resources available to maternity care Veterans including Women, Infants, and Children Programs.
  • Serves as the Mammogram/PAP/Maternity coordinator for Philadelphia HCS. Supports the Women's Health Program and ensures that women have timely coordinated access to all gender specific services.
  • Serves as liaison to Office of Community Care, helping to expedite and troubleshoot access to care in the community for women Veterans.
(1) Patient demographics;
(2) Number of patients being managed for maternity care;
(3) Pregnancy outcomes (maternal complications, neonatal complications, method of delivery and medications) and lactation status.

Requirements

Conditions of Employment

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Preferred Experience:
  • 3-5 years of clinical experience related to patient care unit assignment
  • Evidence of core training in specialty
  • Current AHA BLS
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Physical Requirements: Heavy Lifting (45 lbs. and over); Heavy Carrying (45 lbs. and over); Straight Pulling (up to 1 hour); Pushing (1 - 2 hours); Reaching above shoulder; Use of fingers; Both hands required; Walking (up to 6 hours); Standing (up to 8 hours); Repeated bending (up to 1 hour); Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16"; Hearing (aid permitted).
