Guardian Healthcare is currently seeking a Registered Nurse Assessment Coordinator (RNAC) to assist 3 facilities in our Pittsburgh Market.

This position would float between

Havencrest Healthcare and Rehabilitation Center in Monongahela;

Eldercrest Healthcare and Rehabilitation Center in Munhall; and
Meadow Crest Healthcare and Rehabilitation Center in Bethel Park

What will you do as a Registered Nurse Assessment Coordinator (RNAC)

• Evaluate and identify changes in our residents' conditions and initiate appropriate steps.
• Prepare assessments for our residents at our facility to ensure the services they receive are appropriate and effective.
• Ensure compliance with Medicare and Medicaid programs.

What you need as a Registered Nurse Assessment Coordinator (RNAC)

• Licensed as an RN
• Previous MDS experience
• Previous experience in a long term care facility
