Job Title: Registered Nurse (RN) Rehabilitation Nurse
Job Summary:
The Registered Nurse (RN) Rehabilitation Nurse is responsible for providing specialized nursing care to patients undergoing rehabilitation treatment and promoting their overall well-being. The primary focus of this role is to assist patients in recovering from various injuries or illnesses and regaining their functional independence through personalized care plans and therapies.
Responsibilities and Duties:
1. Implement and monitor individualized care plans for patients undergoing rehabilitation treatment.
2. Assess the patient's physical, emotional, and psychological needs to determine appropriate care interventions.
3. Administer medications, monitor vital signs, and provide wound care as necessary.
4. Collaborate with a multidisciplinary healthcare team to coordinate patient care and ensure continuity of services.
5. Provide education to patients and their families about the rehabilitation process and techniques for self-care at home.
6. Monitor the patient's progress, document observations, and adjust care plans accordingly.
7. Assist patients in completing exercises and therapeutic activities to promote strength, mobility, and independence.
8. Evaluate the need for medical equipment or assistive devices and ensure safe usage by patients.
9. Participate in patient discharge planning, including arranging home care services or coordinating referrals to other healthcare providers.
10. Maintain accurate and up-to-date medical records and ensure compliance with all relevant legal and ethical standards.
11. Adhere to infection control protocols and maintain a safe and clean working environment.
12. Provide emotional support and reassurance to patients and their families throughout the rehabilitation process.
13. Stay updated with advancements in rehabilitation nursing practices and contribute to continuous quality improvement initiatives.
14. Collaborate with community organizations and resources to support patients in their transition back to their daily routines.
15. Adhere to professional nursing standards and code of ethics to ensure optimal patient care delivery.
Qualifications and Skills:
1. Valid RN license issued by the state regulatory board.
2. Bachelor's degree in Nursing (BSN) preferred; Associate's degree in Nursing (ASN) accepted.
3. Proven experience in rehabilitation nursing or related field.
4. Sound knowledge of rehabilitation procedures, techniques, and evidence-based practices.
5. Excellent assessment and critical thinking skills.
6. Strong communication and interpersonal abilities for effective patient and family interaction.
7. Ability to prioritize tasks and manage time efficiently in a fast-paced environment.
8. Proficient computer skills and experience with electronic medical record systems.
9. CPR certification and knowledge of basic life support techniques.
10. Ability to work collaboratively in a multidisciplinary team.
11. Attention to detail and ability to maintain accuracy in documentation and reporting.
