An active RN license

Responsibilities
Conducting phone conversations with patients and collaborating with other healthcare providers
Skillful communication and effective care coordination for patients managing renal disease and multiple chronic conditions
Independently evaluating urgent health issues and promptly relaying concerns to the relevant medical professionals
Delivering patient education to promote informed healthcare decisions

Position Details
Job Type: Full-time, Monday to Thursday, 8:30 AM to 5:00 PM, and Fridays 8:30 AM to 4:30 PM
Compensation: $26.00 - $30.00 per hour
