Registered Nurse - Remote | Wfh

We are seeking a passionate and compassionate healthcare professional to join our dedicated team as a Care Coordinator for our esteemed medical facility. As a Care Coordinator, you will be responsible for assessing and addressing the physical, emotional, social, and spiritual needs of patients, while ensuring they receive the best possible care and support throughout their journey.
Responsibilities
Assessing the physical, emotional, social, and spiritual needs of patients and evaluating their adaptation to health changes.
Developing personalized care plans based on patients' diseases, symptoms, and response to treatment.
Educating patients, families, and caregivers about disease processes, treatment options, side effects, and symptom management.
Collaborating with an interdisciplinary team to ensure seamless continuity of care.
Making appropriate referrals to other clinical resources and professionals, such as social work, home health care, and clinical nutrition.
Implementing, evaluating, and modifying care plans to meet patients' evolving needs.
Integrating research and evidence-based knowledge into your clinical practice.
Building long-term relationships with patients and their families, providing guidance and support throughout their disease and treatment journey.
Assessing patient and family needs throughout the continuum of care, from diagnosis to treatment and survivorship.
Acting as the primary point of contact for patients and caregivers, offering education, triage, and symptom management both in-person and over the phone.
Coordinating care within our medical facility and collaborating with external healthcare facilities.
Providing comprehensive care management for a panel of Heme patients, including care coordination, assessments, telephone triage, and patient education.
Qualifications
Required:
Nursing degree and a current Washington State nursing license.
Current BLS AHA Health Provider Card (or equivalent certification) with renewal every two years.
Critical thinking skills and the ability to analyze, apply standards, seek information, and reason logically.
Development of clinical judgment and the ability to make informed decisions.
Excellent time management skills, demonstrating the ability to organize and prioritize tasks independently.
Effective communication skills, including active listening, asking open-ended questions, and providing clear and empathetic responses.
Proficient information-sharing skills, including using clear and concise language, avoiding jargon, and checking for understanding.
Preferred
Bachelor of Science in Nursing.
Experience in outpatient oncology settings.
Oncology certification.
Two years of nursing experience.
