Registered Nurse / Health Coach

Works under the supervision of UFP leadership. Serves in an expanded nursing role to collaborate via telemedicine outreach with a select group of high-risk patients to optimize medical outcomes. Closely works with patients through recurrent telemedicine outreach to provide a model of care that delivers quality, efficient and cost-effective health care services. Collaborates with the patient's primary care provider and specialists, when applicable, to develop and encourage patient care plans. Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status. Integrates evidence-based clinical guidelines, preventive guidelines and protocols in the development of treatment plans. Develops systems of care and creates patient outreach opportunities that monitor patient progress and promote early intervention in acute care situations. Works effectively through telemedicine services with other members of the health care team to optimize interventions.

Minimum Education and Experience Requirements:

• Three to five years' experience in clinical nursing and case management.
• Critical thinking skills and the ability to analyze complex data sets required.
• General use of computer knowledge and capability to use computers, specifically telemedicine platforms like zoom, required.
• Licensed as an RN in Florida.
• Demonstrated ability to function effectively in a fluid, dynamic and rapidly changing environment.
• Demonstrated ability to function effectively in a telemedicine platform of care.
• Knowledge of the basic concepts and principles of managed care are required.
• Demonstrated ability to influence and negotiate individual and group decision-making.
• Demonstrates customer-focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, patients and families with diverse opinions, values, and religious and cultural ideals.
• Telemedicine experience preferred.
• Case Manager certification preferred.
• Demonstrates leadership qualities, including time management, verbal and written communication skills, listening skills, problem-solving and decision-making skills, priority setting, work delegation and work organization.
• ADN required; BSN preferred. Bachelor's degree in health care preferred.
• Demonstrated ability to work autonomously and be directly accountable for practice.

Licensure/Certification/Registration:

• OCN strongly preferred.
• Current licensure by the State of Florida as a registered nurse.
• Basic Life Support certification or obtained at first opportunity with employment.

Motor Vehicle Operator Designation (Required)::
Will not operate vehicles for an assigned business purpose
