Job Summary
North Central Nursing Clinics (NCNC) is a federally qualified health center (FQHC) providing access to high-quality primary and behavioral healthcare services for the communities surrounding Burlington, Delphi, Monon, and Wolcott.
Under the supervision of the Director of Quality, this position collaborates with the Director of Behavioral Health Services and Behavioral Health team, to serve as the primary Referral Coordinator for NCNC’s Behavioral Health team.
This position works with a team of Referral Coordinators to manage and coordinate referrals for patients with health care needs that are not available within the normal scope of services of NCNC. This person has supervisory responsibility for staff assisting with the process
Core functions of this role include:
- Participates in regular staff meetings and training
- Communicating all relevant referral activity and progress to patients, providers, and partners and save documents all coordinated care efforts in the patient’s electronic health record. This includes utilization of NCNC’s referral tracking application.
- Responsible for keeping the list of referral partners and processes active and up-to-date.
- Provide additional assistance to the Referral Coordination Team, as needed.
- Management and coordination of all internal and external behavioral health referrals, including the completion of all applicable Prior Authorizations and other required documentation per payors
- Identify, communicate, document individual partner needs and processes to the NCNC team.
Other Duties/Misc.
- Comply with all federal and state laws, NCNC policies, procedures, rules, and regulations
- All other duties as assigned
- Attend meetings and training sessions, as required
- Comply with all HIPAA rules
- Represent the organization at conferences and seminars as needed
- Represent NCNC in a professional and courteous manner
Required:
- 4 years of experience.
- Must demonstrate accuracy, proficiency, and efficiency in documentation of referrals.
- Will accept combinations of related education and experience, including degree in Nursing, Social Work, Sociology, Psychology or related Health field in a medical setting.
- Respecting and maintaining confidentiality are critical.
- Must have ability to organize and prioritize work appropriately.
- Must demonstrate tact, patience and excellent oral/ written communication skills in dealing with patients, peers and external partners.
- Should have a strong commitment to the mission of North Central Nursing Clinics and the patients served and demonstrate positive customer service skills.
- Must be able to perform effectively as a member of a multidisciplinary team.
- High school diploma/GED
- Effective analytical and problem-solving skills
Preferred:
- Experience in case management or community services preferred, along with an understanding of the health care delivery system.
- Experience with Medicaid, Medicare and other third-party payers
- Computer knowledge required and experience working in an electronic health record preferred.
