LIFE at St. Francis Healthcare , a member of Trinity Health PACE, is seeking a full-time Recreation Assistant for its location in Wilmington, Delaware.
The primary purpose of this position is to assist with planning, implementation, and evaluation of therapeutic recreational activities for all participants, in accordance with the participant's assessment and care plan. The Recreation Assistant will proactively identify, develop and execute solutions to enhance the participant experience.
This position will work onsite Mon - Fri 8 am until 430 pm with no nights, weekends or on call shifts required.
Programs of All-Inclusive Care for the Elderly (PACE) or LIFE programs coordinate and provide all needed preventive, primary, acute and long-term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today’s fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.
Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

General Responsibilities:
- Monitor recreation therapy supplies and equipment inventory as needed to ensure adequate supplies.
- Plan and implement group and individual activities to improve or maintain the participants' cognitive, physical, social, psychosocial, and artistic needs.
- Assist participants to improve their daily life by promoting socialization, expanding personnel interests, increasing physical activity and continue lifelong learning.
- Assist in providing recreational education and training to support other PACE Center staff members, as appropriate.
- Plan and implement activities for dementia-specific population. Activities may focus on small groups, one-on-one programming and sensory stimulation.
- Develop and maintain a good working rapport with other departments within the program to assure that daily activities program can be performed without interruptions.
- Assist in developing a written plan of care for each participant's activities program that identifies the strengths/needs of the participants and the goals to be accomplished for each opportunity identified.
- Assist in development of a monthly calendar of activities.
- Develop and implement a quarterly newsletter for participants and caregivers informing them of events and on goings of the PACE Organization.
Minimum qualifications:
- Well-developed communication skills, written, oral or through ASL, that may be used either in an on-site or virtual environment is required.
- Education or training in geriatrics, including dementia preferred.
- Ability to work with minimal supervision and exercise independent judgment.
- Must be action-oriented, have business acumen, manage conflict well, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility.
- Able to communicate effectively with individuals and groups representing diverse perspectives.
- Demonstrates superior written, verbal and/or American Sign Language (ASL) communication and presentation skills appropriate for audience comprehension.
- High School Diploma or equivalent required
- Minimum of one (1) year of documented experience working with a frail or elderly population or if less than one (1) year of experience, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
- Position may require 10% travel within the Centers geographic region.
