The role you’ll contribute:

The Record Integrity Auditor/Editor must be a highly skilled MT with proven experience in the medical transcription profession. He/she must have demonstrated fair and unbiased judgment with the ability to review and coordinate a random review process of documents. This includes the process of monitoring, measuring, and reporting transcribed documents by vendor transcriptionists and documents reviewed by their editors. Must be able to analyze transcription data and prepare reports as requested.

The Record Integrity Auditor/Editor is responsible for ensuring the transcription vendor quality and turnaround times are in accordance with contractual agreement. She/he will also be responsible for performing note type changes, note redactions and for moving, deleting, and restoring transcribed documents.

The Record Integrity Auditor/Editor performs inter-facility and intra-facility duplicate medical record combines. Performs electronic medical records reviews to validate the appropriateness of combining medical records or encounters. The Record Integrity Auditor/Editor is also responsible for printing queries/reports to identify new duplicates. Provide medical record combine services to AdventHealth facilities upon request. Audits medical records for completeness before these are released to AdventHealth Facilities Risk Managers or Corporate Legal Counsel. Perform release of information functions for closed facilities and physician’s practices that were previously owned and operated by AdventHealth.

The value that you bring to the team:

• Direct efforts toward quality documentation, including performing QA on a percentage of documents transcribed/edited by vendor transcriptionists, according to AdventHealth guidelines requiring a 98% efficiency rate.

• Through a standard random selection process, select randomly transcribed/edited reports for review. This includes correcting reports deficiencies such as blanks.

• Review the transcribed report against actual dictation, applying industry-specific standards provided by current resources and references.

• Using preferred standard quality scoring criteria, calculate and score reports consistently and fairly, weighing the varying degrees of errors against the documentation length.

• Provide timely and consistent feedback to the AdventHealth HIM Corporate Manager/Director on vendor quality and medical staff feedback.

• Randomly review transcribed/edited documents for adherence to the style guide set up by AdventHealth Advisory Team. Failing to follow these style guides will result in failure of obtaining speech recognition for those providers and therefore will interfere with TAT and productivity.

• Recognize, interpret, and evaluate inconsistencies, discrepancies and inaccuracies in the medical dictation, and appropriately clarify and/or report them as required.

• Monitoring M Modal Fluency Manager application to identify potential delays and address these with the appropriate Corporate/Vendor Representative.

• Consistently review accounts on pending and hold list and makes appropriate report corrections. Perform additional research in the AdventHealth Health Record when incorrect demographic/medical information is provided by the dictator.

• Ensures pre-admission documents that do not contain a medical record number or financial numbers are corrected in a timely manner.

Qualifications

Preferred qualifications:

• Bachelor’s in health information management or related field with 2 or more years of HIM Release of Information experience.

• CMT – Certified Medical Transcriptionist or RMT- Registered Medical Transcriptionist

• Medical Record Combine Experience.

• RHIA, RHIT or

• CMT – Certified Medical Transcriptionist or

• RMT – Registered Medical Transcriptionist

• Associates degree in Health Information Management
