Education and Experience : Bachelor's Degree or higher in Social Work or a related field and a minimum of four (4) years of direct contact with the public or patients in a health program including one (1) year of supervisory or lead work experience.


Licenses and Certificates : Valid Texas Class "C" Driver's License or equivalent from another state by time of appointment.


General Purpose


Under general direction, supervise individual and group work performance in organizing, promoting and conducting specific public health related outreach, activities, services, education and programs.


Typical Duties


Oversee assigned public health program operations. Involves: Provide guidance, technical assistance, and training for team; oversee coordination and logistical needs for outreach events with Health Equity navigators, educators and medical team; create and maintain relationships with community-based organizations, and other health and human services in El Paso County.


Promote and conduct health navigation services. Involves: Conduct intake interviews with clients/patients to identify individual needs. Link individuals to programs and resources in the community as needed. Stay informed of changes in the healthcare organization and available patient resources. Keep abreast of current disease information. Conduct case management activities to include eligibility determination, enrollment and follow-up with patient to ensure timely treatment. Assist patients with completing applications and registration forms. Provide referrals for services to other community agencies as appropriate including emotional, financial, administrative, educational, or cultural support services. Prepare and maintain client files in accordance with HIPAA regulations.


Serve as a technical resource for the program and/or clinic. Involves: Work collaboratively with health providers and organizations to facilitate client/patient healthcare navigation. Identify community needs and recommend actions. Monitor and analyze customer contacts. Provide information about patient navigator services and locate at-risk patients in need of patient navigation services. Organize educational community engagement and outreach information to promote programs to communities. Facilitate public health meetings throughout the community. Gather input from other social agencies, schools and committees for collaborative efforts.


Supervise assigned staff. Involves: schedule, assign, instruct, guide and check work. Appraise employee performance. Provide for training and development. Enforce personnel rules and regulations and work behavior standards firmly and impartially. Counsel, motivate and maintain harmony. Interview applicants. Recommend hiring, termination, transfers, discipline or other employee status changes.
