Attend and participate in regularly scheduled case reviews with care coordinator and accountable care manager.
Education: High School Diploma; Associates preferred.
Relationships: Ability to build and effectively manage relationships with business leaders and external constituents.
Complies with quality assurance, OSHA, HIPAA, patient centered medical home focus, infection control, safety and other policies set forth.
Work directly with CINQCARE’S Family Members, care coordinator, family/caregivers, and discharge planners to assist in transition planning.
The PHN will use Motivational Interviewing stages of change model.
