Job Description
This position is hybrid and includes work in the office and in the field covering Polk, Highlands, and Hardee counties.


Job Goal: To improve/increase the prenatal and infant screening rates to maximize program participation. Engage medical providers, professionals, and hospitals to support the Healthy Start mission.


DUTIES INCLUDE


  • Attend Health Fairs, Provider Fairs and other events to represent the Healthy Start Coalition and Healthy Start Services. Some events may be held on Saturday.
  • Develop and produce newsletters, quarterly for providers, hospitals and medical professionals and semi-annually for Mommy Matters and Mommy Knows Best.
  • Routine use of the Well Family System, which is a complex case management system, to review data, pull reports, and track Healthy Start services to present data in newsletters or to providers.
  • Ensure Healthy Start Coalition brochures are current and are placed in relevant locations, i.e., provider offices, hospitals, WIC offices, etc.
  • Liaison between providers, hospitals and internal Central Intake and Referral (CONNECT) Supervisor.
  • Visit OB providers, hospitals, birth centers, pediatricians, and medical professionals as required by contract. Ensure each visit includes an awareness or educational training.
  • Perform other incidental tasks or services consistent with the job goal of this position, assist with other duties as assigned.
  • Ensure OB providers have prenatal risk screens and are trained on their use. Work with FDOH Polk to ensure screens received have been completed accurately and are received in a timely manner. Working with FDOH Polk to monitor screen compliance.


Desired Qualifications


  • Ability to travel in Hardee, Highlands, and Polk Counties
  • Proficient is MS Office and other computer-based applications
  • Minimum of Associate's degree, with knowledge of community relations, public health issues, social determinants of health, and maternal and child health experience preferred
  • Reliable transportation and current automobile insurance coverage
  • Interpersonal skills require courtesy, tact, confidentiality, and empathy always, to communicate effectively and convey information to clients, business partners and co-workers.
  • Valid Florida Driver's License

  • Excellent English language verbal and written communication skills.
  • Experience working in Healthy Start and/or familiarity with Healthy Start System of Care.

  • Excellent written and verbal communication skills as well as strong client engagement skills.
  • Upon hiring, candidate must have an AHCA level 2 background screening and drug test. Report results may impact ability to be employed.
  • Proven team player.
  • Must be able to work independently and think creatively.
  • Ability to effectively organize work activities and complete tasks in a timely manner.
