Job Description

General Summary of Position
Assists with the overall operations of Population and Community Health programming including planning, communication, logistics, implementation, and documentation. Will work closely with staff across the organization, system, and community, including but not limited to government agencies, faith-based institutions, and non-profit organizations.


Primary Duties and Responsibilities

  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts.
  • Builds and maintains relationships with community partners.
  • Staffs community events and activities as needed.
  • Collects, tracks and reports information about the community and community health programming and community benefits.
  • Facilitates health/wellness/fitness programming as needed.
  • Performs other duties as assigned.
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Develops and maintains procedural and training manuals for programs, equipment and facility maintenance protocols and scheduling.
  • Participates in multi-disciplinary quality and service improvement teams.
  • Assists in the selection, training, orienting and assignment of volunteers for community events. May assist with development of standards of performance management planning.
  • Participates in the planning, execution, and analysis of community health needs assessments.
  • Plans, manages, and documents community health event logistics with multiple stakeholders.


Minimum Qualifications
Education

  • Bachelor's degree or an equivalent combination of education and related experience. required

Experience

  • Participation with a community health organization. experience as a lay facilitator in health/wellness programs. preferred
  • 1-2 years 2 years related experience required and

Licenses and Certifications

  • Certification as lay facilitator in health, wellness, fitness Upon Hire preferred and
  • Must obtain lay facilitator certification within 6 months of hire. within 180 Days required

Knowledge, Skills, and Abilities

  • Teaching skills to promote healthy behavior change.
  • Strong working knowledge of MS Outlook, Word, and Excel software applications.
  • Basic computer skills required.
  • Verbal and written communication skills.
