Homelessness is the largest social and public health crisis in California. Illumination Foundation (IF) is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children's and family programs. IF currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

The Primary Care Physician will work with the Medical Director in successfully leading the

performance of the clinical practice by providing excellent and comprehensive medical services to

the Medical Group. This is a per diem role with a pay rate of $115 per hour.

Responsibilities:

Clinical Activity and Leadership:

• Potential back up, and after hours phone call
• Potential supervision of Nurse Practitioners
• Documentation in the Electronic Medical Records
• Provision of direct patient care

Clinical Performance, Quality and Leadership Accountability

• Quality metric performance on a per-provider and program basis
• Client Access and clinical capacity management in alignment with the strategic performance goals of the primary care continuum of IFMG.
• Accountable for clinical practice adherence to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to IFMG policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.
• Accountable clinical supervision to medical assistants, front desk staff, and support staff
• Accountable for the clinical performance of the site-specific medical provider team. Clinical performance includes but is not limited to these specific priorities.
• Clinical Documentation Timeliness.
• Clinical Documentation and Coding Accuracy.
• Consulted in the development of medical practice guidelines, policies and protocols

Minimum Qualifications:

• Excellent communication skills including experience with de-escalation, trauma-informed communication and demonstrating a history of effective “bedside manner” with patients.
• Ability to deliver high levels of customer service and achieve customer satisfaction
• High attention to detail and ability to focus
• Knowledge, education, training, skills, and experience in a multi-disciplinary approach to physical and mental health and chemical dependency.
• Understanding of compliance and regulatory guidelines (HIPAA, OSHA, etc.)
• Experience in established leadership capacity within a healthcare setting strongly preferred.
• License to practice as a NP, PA,DO or MD in California
• Demonstrated leadership ability to build and sustain effective medical provider teams, provide and communicate vision, and effectively manage change.
• Knowledge, training, skills and experience in providing and supervising services in a highly diverse work environment to a highly diverse and mostly homeless population.
• Minimum two years’ experience in providing medical care as a medical provider for underserved individuals in a community clinic setting.
• Residency trained in a Primary Care Training program
• Computer proficiency (MS Office, Google Drive, EMR, Billing Systems)
• Demonstrated ability and accomplishment in continuous clinical quality performance improvement and practice innovation.
• Demonstrated skills in written, verbal and consultative communications
