Position Summary:


We have an exciting opportunity to join our team as a Per Diem - Physical Therapist.


In this role, the successful candidate Evaluates, tests and treats inpatients as per referral. Develops comprehensive therapeutic plans, documents patient progress daily, revises treatment plans when appropriate, educates patient on disease process and is involved with discharge planning. The therapist uses all physical therapy modalities and techniques. Provides lectures and demonstrations to the hospital staff and affiliations as requested.


Job Responsibilities:


  • Seeks information regarding quality of care rendered by self and others under their supervision.
  • Legibly documents in the patient’s chart, on departmental forms, and any other hospital forms all aspects of Physical Therapy care. This is to include screening examination, evaluation, response to treatment, discharge planning, family conferences and communication with other health care professionals.
  • Fully completes departmental forms such as the Physical Therapy Initial Evaluation form and Physical Therapy re-evaluation form.
  • Promotes teamwork and customer satisfaction.
  • Selects intervention strategies to achieve the desired outcomes.
  • Demonstrates a willingness to alter schedule to accommodate patient needs and facility requirements.
  • Is sensitive to the psychosocial needs of patients and their families.
  • Practices in a safe manner that minimizes risk to patient, self and others. Demonstrates awareness of contraindications and precautions of treatment.
  • Discusses treatment plan and/or associated problems with physicians and nursing staff.
  • Promotes the profession of Physical Therapy.
  • Exhibits sensitivity to differences in race, creed, color, gender, age, national or ethnic origin, sexual orientation, and disability or health status.
  • Documents any patient or family education on the hospital Patient and Family Education form.
  • Wears attire consistent with expectations of the hospital. Wears hospital badge at all times.
  • Demonstrates a positive attitude, positive attitude towards change, positive customer service skills, demonstrates behaviors that contribute to a positive work environment.
  • Selects relevant information to document the delivery of physical therapy patient care.
  • Coordinates physical therapy with other services to facilitate efficient and effective patient care.
  • Accepts assignments willingly.
  • Initially and ongoing, reviews medical record, observes and interviews patients, reviews medical histories including physicians’ diagnoses and orders.
  • Follows the established Policies and Procedures and Safety precautions in the use of all modalities
  • Follows established guidelines for the delivery of physical therapy services. (eg, critical/clinical pathways, protocals)
  • Maintains knowledge of current trends and techniques by attending related conferences/seminars, reading appropriate journals, books etc. Participates in inter and intra-departmental instruction.
  • Displays good time management skills, integrity, respect, caring for others, accountability, positive interpersonal, and communication skills, possesses the ability to constructively give and receive constructive feedback, and problem solving.
  • Performs and documents all initial evaluations and progress notes in a timely fashion.
  • Adjusts the plan of care in response to changes in patient status.
  • Develops and implements plans for self-improvement.
  • Organization of workload to meet deadlines and priorities established by management.
  • Assists in the supervision of Physical Therapy Assistants, the Rehabilitation Aide and the Rehabilitation PSA.
  • Treats internal and external customers with a positive regard, dignity, respect and compassion.
  • Explains treatment procedures to patients and/or family members to alleviate fears and elicit cooperation.
  • Maintains patient privacy, modesty and confidentiality.
  • Demonstrates behaviors that contribute to a positive work environment.
  • Examines equipment prior to patient use.
  • Participates in Performance Assessment and Improvement program.
  • Establishes short- and long-term goals in conjunction with the patient and formulates treatment plans consistent with these goals.
  • Provides recommendations for developing or modifying guidelines based on outcome measures, effectiveness studies and clinical observations.
  • Is punctual and dependable.
  • Covers all Physical therapy areas. Identifies clinical limitations and seeks guidance as necessary to address limitations.
  • Determines the appropriate equipment and modalities required to achieve goals.
  • Produces documentation that follows guidelines and format required by the practice setting.
  • Voluntarily helps other staff members.
  • Determines physical therapy related tasks that can be legally and ethically delegated.
  • Takes part in departmental program planning and development.
  • Willingness to take on and develop new projects and services.


Minimum Qualifications:


  • New York State Physical Therapist license or eligible for licensure with current registration.
  • Current basic life support
  • Adapts delivery of physical therapy care to reflect respect for and sensitivity to individual differences.
  • Graduate from an approved Physical Therapy curriculum with a minimum of a Bachelor of Science Degree.
  • Demonstrates professional behavior during interaction with others.
  • Ability to walk and stand for up to 75% of work time and lift and turn patients.
  • Communicates in ways that are congruent with situational needs.
  • Ability to focus on patient needs and detail 75% of work time.
  • Practices in a safe manner that minimizes risk to patient, self and others
