Job Description
Description:

A. This position is located within Raleigh Radiology imaging centers, free-standing outpatient imaging centers located throughout Wake and surrounding counties, with the ability to work remotely.

B. The Patient Nurse Navigator is a Registered Nurse who serves as a liaison between patients and clinical staff. The Patient Nurse Navigator’s primary role is to facilitate and coordinate patient care throughout the imaging process, ensuring smooth and efficient transitions between different stages of radiology procedures and treatment.

C. The Patient Nurse Navigator is responsible for maintaining mammography patient records, scheduling and tracking mammography follow-up including diagnostic mammography, biopsies, and pathology results, and sending out patient results letters for all Raleigh Radiology Imaging Centers.

D. This position reports to the Senior Manager of Practice Operations

1. Implements, maintains, and follows policies and procedures of Raleigh Radiology.

2. Performs job responsibilities in a safe, efficient, and professional manner.

3. Patient Education: Provide patients with clear and understandable information about tests, procedures, and potential side effects. Answer any questions and address their concerns.

4. Coordination of care: Collaborate with radiologists, referring physician, patients, referral coordinators, and coworkers to schedule and coordinate appointments and treatment to ensure timely and accurate delivery of care.

5. Support and Guidance: Offer support and guidance to patients and their families throughout the process, ensuring they feel informed and empowered.

6. Advocacy: Advocate for patients’ needs and preferences, ensuring that their voices are heard during decision-making processes.

7. Navigation of Services: Assist patients in navigated the process, including directing them to appropriate imaging services and explaining the purpose of each test.

8. Documentation: Maintain accurate and up-to-date patient records and ensure that all necessary documentation is completed.

9. Quality Assurance: Participates in quality improvement initiatives while promoting patient safety and optimal outcomes.

10. Assists with/ or institutes emergency measures for sudden adverse occurrences when necessary such as contrast reaction.

11. Collaboration: Collaborate with other members of the healthcare team to improve patient care and streamline processes.

12. Establishes and maintains open communication.

13. Maintains a clean, safe, and well-maintained environment for patients and staff.

14. On regular, sustained basis, cooperates with other staff members both inside and outside the department in accomplishment of own job duties as well as assisting other in accomplishing theirs; serves as team player and role model for other employees in the organization, always exhibiting traits of courtesy, caring, helpfulness and respect; conducts oneself in service-oriented manner that is attentive, pleasant, cooperative, sensitive, respectful and king when dealing with team members, visitors, public and all employees.

15. Is properly trained and can demonstrate competent use of Radiology Information System (RIS), telephone scanners/printers, other equipment related to the position.

16. Performs other duties and accepts responsibility, as assigned.

Job Specific Duties 70%

1. Performs follow-up calls and updating reports with pathology results following breast biopsies and lung study patients.

2. Ensures patients are following up with breast and lung recommendations.

3. Ensures smooth transitions between care modalities, facilities, and providers.

4. Contacts patients and referrers to schedule breast biopsies and obtain all orders and documentation needed prior to performing procedure.

5. Conducts outreach to referrers, providers, and other medical professionals as well as to corporate “customer.”

6. Executes MRI prostate segmentation workflow.

7. Performs all tasks required to track, print, and mail results letters to patients for mammography within MQSA guidelines.

8. Assists with QC Specialist with any additional duties related to ACR, MQSA, or patient safety tasks as assigned.

9. Answers telephone calls by 3rd ring to accomplish records requests from referring physicians’ staff, referring physician, and patients.

10. Follows established protocol for providing service in a professional, efficient manner.

11. Answers questions from patients in scope of knowledge. Directs questions to appropriate personnel if unable to answer questions.

Requirements:


A.
Education: Registered Nurse, CPR Certified (Cardiopulmonary Resuscitation)

B. Experience: Must have experience in a medical setting. Preference given to applicants with previous experience in Radiology practice

C. Licensure: CPR and Current Registered Nurse

Ability to work independently, make sound decisions based on patient condition and ability to follow directions. Flexibility, dependability, and professionalism required.

III. WORKING CONDITIONS / PHYSICAL REQUIREMENTS

Must be able to read without strain printed material the size of typewriter characters, glasses permitted, and the ability to hear the conversational voice, with or without a hearing aid, or the ability to compensate satisfactorily. Frequent, prolonged periods of sitting. 100% of work is done at computer. Ability to type at 60+ wpm. Wears headset to connect to phone. Subject to rotating holidays or shifts.
