The HIV Patient Navigator provides case management and care coordination services to assist patients in identifying and overcoming barriers to engaging in HIV medical care and achieving viral suppression. This position also provides education for individuals at risk for HIV infection.

We Require


  • Two years of work experience as a social worker, patient navigator, case manager, or health educator in a healthcare environment
  • Combination of post-secondary education and/or job-related experience may substitute for the minimum qualifications on a year-for-year basis
  • Bachelor’s degree in social work or related field


We Also Value


  • Experience providing HIV services
  • Bilingual in English and Spanish
  • Specialized training or experience in HIV prevention interventions, PrEP/PEP, and other sexually transmitted diseases


Job Contributions


  • Perform follow-up risk assessments to reinforce the patient's realistic perception of risk
  • Provide access to prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance
  • Develop PrEP/PEP resources available in the area for the patients in need of services
  • Provide confidential patient assessments to determine the HIV prevention needs of the patient
  • Collaborate with the HIV team for linkage and relinkage to prevention services, sexually transmitted disease screening, and follow-up


Working Conditions


  • Within one month of hire, all employees are required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health; required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza; some positions require a Hepatitis B vaccine and/or a Tuberculosis test
  • Must be able to participate in emergency response training as scheduled and be available to respond as directed in the event of a public health emergency
  • Requires travel to and from job-related locations during a scheduled workday, subject to County policies regarding the use of County or private vehicles on County business
  • Arizona driver’s license upon hire
