Job Description
Citadel Home Care is a leading provider of high-quality, individualized home health care services. We are currently seeking a Patient Coordinator to join our team. The Patient Coordinator will be responsible for managing all aspects of the placement process for clients, ensuring that they receive the best possible care and support. This is an exciting opportunity to work in a fast-paced, dynamic environment and make a difference in the lives of others.


Responsibilities


  • Provide support to caregivers as needed
  • Create and manage schedules for home care providers
  • Ensure that all necessary documentation is completed accurately and in a timely manner
  • Conduct thorough assessments of clients' needs and develop customized care plans
  • Maintain strong relationships with clients, families, and other stakeholders
  • Collaborate with healthcare professionals to ensure seamless transitions of care for clients
  • Monitor the quality of care provided, making adjustments as needed to meet clients' needs


Requirements


  • At least 3 years of experience in home health care or a related field
  • Ability to work independently and manage multiple tasks simultaneously
  • Passion for providing high-quality care to clients and their families
  • Strong understanding of home care regulations and requirements
  • Strong organizational and time management skills
  • Excellent communication and interpersonal skills
