Position: Patient Advocate (PAC Team)


Location: Memphis, TN

The Day-to-Day:


The Patient Advocate builds trust and fosters relationships with the older adults living within Matter Health communities. This role is responsible for driving new patient visits to the clinic and enrolling patients in Matter Health Membership.


Focused on driving clinic utilization and championing membership, the Patient Advocate, will partner closely with our clinic staff to execute the scheduling and management of events. You should strive to provide exceptional events tailored to the communities we serve and possess a desire to enhance the quality of life for residents.


Your primary responsibilities are building awareness for Matter Health’s value and services in the community and increasing the number of residents that choose Matter Health as their Primary Care Provider. A few ways this can be accomplished are by coordinating and managing on-site events, door to door engagement, 1:1 meetings and more. Whether engaging with individuals in the communities we serve, organizing events to bring residents together, or working with clinical partners to coordinate additional services, you are an active & visible member of the community. 


  • Maintain strong relationships and regular communications with property staff
  • Maintain the Matter Health value proposition and champion benefits to potential members
  • Schedule initial Clinical Intake Visits in Athena
  • Partner closely with our clinic staff to execute the scheduling and management of the event calendar and directly aid our team in growing our member base
  • Schedule and conduct meetings with leads to share our mission, vision, value proposition and services
  • Identify and coordinate with local vendor partners and leverage channels to increase PCP conversions – including building staff and other care providers
  • Enroll individuals living in our communities to become patients and members of a Matter Health clinic
  • Manage community-based vendor partnerships and on-site logistics to enhance the experience of our residents and patients
  • Develop and execute lead generating strategies, producing a monthly calendar of events to ensure growth in patient registration and Matter Health Membership
  • Coordinate and lead community events to increase awareness, trust, and understanding of Matter Health
  • Learn to use marketing and sales tools to identify solutions and engage new patients; Maintain up-to-date patient information in designated customer relationship management (CRM) tool (i.e. Athena and Salesforce)
  • Use Matter Health marketing materials to promote scheduled community events
  • Attend all resident events in the assigned market and lead day of event setup and take-down
  • Ensure retention by maintaining relationships with members and guarantee Matter Health is delivering on value
  • Provide content to PAC Team leaders for property monthly newsletter
  • Manage member onboarding packet distribution and inventory
  • Oversee event expenses, budgeting and supply ordering
  • Document the attendance and resident engagement metrics at each event in Salesforce
  • Attend annual training and company events to collaborate with other teammates across different market segments
  • Collaborate with the Patient Advocate leaders to develop PCP conversion strategies to improve market share in all product lines
  • Intake resident and clinical feedback to enhance the event schedule and increase engagement/attendance
  • Performs other duties as assigned
  • Collaborate with insurance and distribution partners who help patients understand their current benefits and enroll in a new insurance if needed
  • Collaborate with property staff on all events hosted by both the property and Matter Health


Requirements


What Leads to Success?


  • Excellent interpersonal and problem-solving skills. Must have the patience and ability to engage leads in conversation
  • Ability to travel throughout the assigned market and comfort with conducting home visits
  • Ability to connect with people and understand the challenges they face
  • Experience with and passion for the older adult population
  • High School diploma or equivalent
  • Detail-oriented and proficient in MS office (PowerPoint, Excel, Word, and Outlook), CRM tools (Salesforce) plus a variety of other word-processing, spreadsheet, database, Outlook and presentation software
  • Valid driver’s license, clear driving records and up to date car registration
  • Effective written and verbal communication skills demonstrating respect and cultural awareness during interactions with residents and members
  • Passion for Matter Health model, vision, and patient population
  • 2+ years relevant sales experience with establishing and maintaining relationship with business/vendor partners
  • Demonstrated knowledge of negotiation, probing skills, closing skills and handling objections

  • Experience with event management and execution
  • General understanding of Medicare Advantage Prescription Drug plans
