Work somewhere that cares! We are looking for Occupational Therapists to work on our fast-paced Bayshore Homecare team.

Job Duties:

• Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload.
• Communicates significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated.
• The Occupational Therapist will assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for occupational therapy and identifies patient needs for other disciplines and services.
• Instructs patient or responsible caregiver in the projected care plan and expected outcomes as directed by the physician's treatment plan, and consistent with Agency regulatory and professional standards.
• Demonstrates effective communication skills, keeping lines of communication open between Lead O.T. Supervisor, Team Managers and all staff involved in care of assigned patients.

Requirement:

• 2 years of experience as an occupational therapist
• Licensure: Current registration to practice Occupational Therapy in New York State
