Job Description
Position Overview:
4- Day Work Week $170,000/annually
Monday - Friday 9 am - 5 pm
Location: Ralph Ave on the C line in Brooklyn

SCOPE
OF ROLE: A Psychiatric Nurse Practitioner on an Assertive Community Treatment (ACT) Team plays the role of the medication prescriber. The PNP is part of a multidisciplinary team that provides comprehensive mental health care to persons served living with chronic mental illness. The PNP will work in collaboration with the RN and LPN to diagnose and manage patients' mental health concerns. The PNP will provide services in the community.

ESSENTIAL DUTIES & RESPONSIBILITIES:
  • Document patients' medical and psychological histories, physical assessment results, diagnoses, treatment plans, prescriptions, or outcomes.
  • Provide crisis intervention and address all emergency situations as appropriate.
  • Meet with Program Director, Assistant Director- Program Nurse, and Social Service staff to coordinate ongoing psychiatric treatment.
  • Maintain CPR/First Aid Certification.
  • Write prescriptions for psychotropic medications as allowed by state regulations and collaborative practice agreements. Review health assessments completed by the Program Nurse and provide clinical feedback.
  • Provide training to ACT team members on topics including medications and drug interactions.
  • Prepare Psychiatric Assessments for the purpose of HRA 2010e Housing Application and/or other applications for housing or needed services.
  • Provide medication management including prescription refills and medication education to consumers.
  • Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, Rules, Regulations, and Statutes.
  • Attend supervision with Medical Director at least monthly.
  • Attend staff meetings, case conferences, group supervision, and trainings as needed.
  • Assist in building evacuation during fire drills and building emergencies.
  • Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff.
  • Communicates and coordinates care with psychiatrists in the community, as needed.
  • Provide in service staff training as needed and consultation as needed to the Act team.
  • Maintain up to date, accurate, complete, and timely consumer chart documentation.

Qualifications:
REQUIRED EDUCATION AND EXPERIENCE
  • Must have a DEA# and an individual NPI #.
  • Must be registered as a NY State Medicaid provider.
  • Licensed as a Registered Nurse in New York State and credentialed by the State to practice as an advanced Nurse Practitioner with a specialty in psychiatry.
PREFERRED QUALIFICATIONS & SKILLS
  • Bilingual preferred.
  • Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
  • Preference is given to those who have experience working on a multi- disciplinary team and/or previous experience with homeless population.
