Nurse Practitioner - Caresight - Weekend On Call

Job summary

The weekend on-call Nurse Practitioner is responsible for providing health care remotely to patients enrolled in the CareSight Program from 8:00 AM Saturday morning until 8:00AM Monday morning

Position Essential Functions:

• Sends an email of the completed on-call templates of all calls received during the stated period to the entire team Monday morning.
• Completes the on-call template for each patient phone call including date and time of call, name of patient and name of person calling (if other than patient), facility if applicable, assigned provider, reason for call and intervention(s) performed.
• Conducts an extensive history of present illness concentrating on the patient’s chief complaint(s) which is done via the telephone conversation.
• Copies and pastes the completed template into the patient’s EMR using the Communication Encounter.
• Delivers high quality care to enrolled CareSight patients that contact Emcara Health after normal business hours via the Answering Service from Saturday morning at 8:00 AM through Monday morning at 8:00 AM
• Develops an assessment (diagnosis) and a plan of care which may include education, ordering medication(s), ordering diagnostics, or recommending a referral to a facility that can provide urgent or emergency care as necessary.

• Experience in care of adult, chronically ill patients
• Credentialed in adult, family, or geriatric care
• Has active DEA license
• Holds active, unencumbered license in state of practice
• Two or more years of practice experience preferred
• Board Certified AANP or AANC
• Driver’s license without restrictions and good driving record
• Current CPR/BLS certification
• Working knowledge of computers and ability to document effectively and efficiently in an electronic system
