Job Title: Nurse Navigator – Oncology, Highlands Ranch

Job Summary:


The Nurse Navigator (NN) will be a member of the care team that will be focused on providing personalized one-on-one patient care. The NN assesses and evaluates patient conditions, and then plans, implements, provides, and documents patient care in a manner that follows professional nursing standards and outlined practices. The NN will assist in expediting the process for evaluation of clinic patients through a multidisciplinary team approach, coordination of services and scheduling exams, optimizing patient care outcomes. The NN will help the patient navigate clinic visits, follow-up testing, diagnostics, labs and other services, coordinates care to reduce gaps in understanding, counsels and educations patients and families, and ensures high quality patient outcomes.

Key Responsibilities:

  • Facilitates resolution to patient care gaps and follows up by both proactively reaching out to patients/families, and continuously addressing direct phone calls and messages from patients.
  • Facilitates patient intake by phone, email or online through Epic MHC.
  • Triages, coordinates care, and orders appropriate patient testing under University of Colorado School of Medicine protocols and guidance.
  • Participates in relevant quality workgroups to assist in the development of patient education materials, clinical pathways, and workflows to improve patient and provider compliance with meeting health measures and meeting the needs of the patients/families.
  • Acts as Liaison between patient and clinical staff, administration, physicians, managed care companies, community/external resources.

Qualifications:


Minimum Qualifications:

  • Substitution: Equivalent professional nursing experience will be considered in lieu of degree.
  • At least three years of nursing experience.
  • Bachelor’s degree in nursing, health care, or related field.

  • Registered Nurse (RN) license in the State of Colorado.

Applicants must meet minimum qualifications at the time of hire.

Conditions of Employment:

  • Current Basic Life Support (BLS) for Healthcare Providers issued by American Heart Association or American Red Cross healthcare provider level CPR certification.
  • Must be able to travel to alternate locations as assigned.
  • Must be able to work in-person.

Preferred Qualifications:

  • Bilingual, Spanish-speaking.
  • Nursing certification (ANCC) in specialty area.
  • RN experience in Specialty Area.
  • 2+ years job-related/clinic experience.

Knowledge, Skills, and Abilities:

  • Ability to work within large, complex healthcare systems.
  • Ability to analyze and interpret data.
  • Ability to compose letters and memorandums.
  • Ability to deal calmly and courteously with people.
  • Ability to collaborate with multidisciplinary teams and respond to time-sensitive situations.
  • Ability to effectively communicate at all levels, including physicians, leadership, and staff within the various affiliate organizations (UCH, Children’s Colorado, CU Medicine, CU SOM).

Anticipated Pay Range:


The starting salary range (or hiring range) for this position has been established as $69,511.

The above hiring range represents the University’s good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation depending on the level.
