Position Summary:


The New York University Grossman School of Medicine World Trade Center Health Program Clinical Center of Excellence (NYUGSOM WTCHP CCE) is one of seven centers established by the James Zadroga 9/11 Health and Compensation Act of 2010 to care for 9/11 first responders.


We have an exciting opportunity to join our team as a nurse case coordinator. This is a unique opportunity to work on a multi-interdisciplinary level on providing care to WTC Responders. Responsible for all aspects of clinical nursing case management and coordination of patients care in the World Trade Center Health Program


Job Responsibilities


  • Coordination with case management for post-discharge outreach
  • Review and case manage members admitted for inpatient (IP) hospitalizations and all other levels of care including rehabilitation, skilled nursing facilities, home health care, any post-acute care facility or service
  • Works with WTC team on care coordination and provide clinical support as needed and requested
  • Attends all weekly, monthly, and semi-annual scheduled client meetings in which clinical based updates or decisions are required
  • Medical necessity review of outpatient procedures and services
  • Review of authorizations for medical necessity (prior authorization/prospective reviews and retrospective authorizations) as related to member’s WTC certified condition(s)
  • Address the unique health care needs of individual members and improve member satisfaction with the Program
  • Discharge planning
  • Develops Intensive case management Plan of Care and necessary follow-ups
  • Assess active members for case-management service needs as clinically indicated
  • Review, complete and submit necessary prior authorizations
  • Ensure that all pre-authorizations issued to enrolled members are for WTC-certified health conditions, or medically associated conditions, in compliance with program guidelines
  • Concurrent (medical necessity) review (CCR) and Continued Stay Review (CSR) processes for IP hospitalizations
  • Care Coordination and Case Management for WTC responders
  • Administering medical monitoring questionnaires

Minimum Qualifications:


To qualify you must have a NYS Licensed Registered Nurse
Must have a Baccalaureate degree in Nursing
BCLS Required
Possesses the necessary education and experience as a registered nurse
Has relevant clinical experience to make independent clinical decisions
Strong organizational and interpersonal skills
Exceptional communication skills with a demonstrated ability to establish and maintain effective working relationships with a wide range of individuals/groups


Preferred Qualifications:


A passion for the NYUGSOM WTC CCE mission and working with adult first responders from varied occupational backgrounds
Prior experience with the WTC population
Prior case management experience
Oncology nursing experience
Spoken mastery of Spanish preferred

NYU Langone Health provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88,978.50 - $126,399.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
