Job Description

Legacy Behavioral Health Center, Inc.is a community mental health center that has been serving residents of Martin, Okeechobee, St. Lucie, Indian River and Brevard Counties since 2005. Our Mission is to serve individuals through their diversity, that is, considering their cultural background, speaking their language, providing mental health services both in office or on site, that is, going to homes, shelters, schools, or others; and keeping respected and fairly treating quality providers.


We are currently seeking a - Full Time / Bilingual - Medical Records Assistant / Case Assignment for our office in Port St. Lucie - St. Lucie County.

Under general supervision, Medical Records Assistant / Case Assignment coordinates the processing of clinical charts to different departments and ensures the proper filing of clients’ information.


  • Record Keeping: Indexing/Filing clients’ clinical and pertinent documentation in file.
  • Assigns all case transfers accordingly.
  • Follows HIPPA confidentiality requirements and Agency's policies and procedures.
  • Enters data as required in Lauris Electronic Medical Records System or determined necessary by supervisor
  • Verify funding prior to assignment of each referral.
  • Follows LBHC safety guidelines.
  • Ensures strict confidentiality by following HIPAA standards and Agency’s policies.
  • Logs all faxed documents.
  • Ensures proper consent forms are filled out and signed in order to release information.
  • Assign Therapy in the field cases by completing the Staff assignment form and notifying the independent contractors and their consultant via email.
  • Locates charts for upcoming psychiatric and outpatient, in-office appointments.
  • Conducts and documents Quality Assurance calls to clients as needed.
  • Maintain Therapy referrals for assignment waiting list.
  • Looks up information in Lauris Electronic Medical Records System.
  • Verify funding monthly for each referral pending.
  • Maintains clinical logs when charts are checked out.
  • Generates correspondence/faxes/emails as required.
  • Ensure/verify that all referrals have the authorizations in the system, if applicable.
  • Works in cooperation with other departments within the agency.

Detailed description will be provided during interview.

  • Bilingual English/Spanish a MUST;
  • Minimum of High School Diploma or Equivalent;
  • Experience with medical records filing and processing preferred.
