Job Summary:

We are seeking a dedicated and compassionate LPN (Licensed Practical Nurse) to join our skilled nursing team. As an LPN in our facility, you will be responsible for providing comprehensive nursing care to residents and ensuring their well-being. Your primary goal will be to assist in maintaining a safe and nurturing environment for our patients while promoting their physical and emotional health.

Responsibilities and Duties:

• Adhere to infection control and safety protocols to maintain a clean and secure environment.
• Document all nursing care activities accurately and timely in medical records.
• Follow facility policies and procedures to ensure compliance with federal, state, and local regulations.
• Assist with activities of daily living, such as dressing, bathing, and eating, while maintaining residents' dignity.
• Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.
• Monitor residents' vital signs and assess their physical and mental health status.
• Educate residents and their families on health conditions, medications, and self-care techniques.
• Provide direct nursing care to residents under the supervision of registered nurses or physicians.
• Participate in staff meetings, continuing education programs, and professional development activities.
• Administer prescribed medications and treatments in accordance with physician orders and established procedures.

Qualifications and Skills:

• Familiarity with electronic medical records systems and basic computer skills.
• Strong attention to detail and critical thinking abilities.
• CPR certification is required.
• Excellent communication, interpersonal, and organizational skills.
• Must hold a current and valid LPN license in the state of [State Name].
• Physical stamina to meet the demands of the job, including standing, walking, and lifting.
• Minimum of one year of experience in a skilled nursing facility or long-term care setting is preferred.
