AS THE LICENSED PRACTICAL NURSE, you'll be under the supervision of the Home Health Supervisor, and the Home Health Licensed Practical Nurse, administers skilled nursing care to patients on an intermittent basis in the patient’s place of residence in close coordination with other disciplines and caregivers involved in the patient’s care. This is performed in accordance with physician orders and plan of care and in accordance with agency policies and procedures.

Qualifications:
  • Have strong written and verbal communication skills.
  • Be able to exercise initiative and independent judgement.
  • Have ability to work well independently and within a multidisciplinary team.
  • Be a Licensed Practical Nurse with a valid PA Nursing License.
  • Have two years of Licensed Practical Nursing experience in acute care or long term care setting preferred.
  • Have strong teaching skills.
  • Have previous Home Health experience
  • Have valid PA driver’s license and reliable transportation.
  • Have excellent time management skills.
