Licensed Practical Nurse, Intake - Home Care

Summary: All referrals to the Home Care department and for the accuracy and completeness of each referral. Assisting the Intake Nurse in all clinical documentation for accuracy and completeness to meet JCAHO and Medicare standards. Obtain managed care and authorizations when appropriate. Monitor blood pressures at designated community clinics and provide flu immunizations at flu clinics.

Education: In addition to the skills normally obtained through completion of a High School education, must be a graduate of an accredited school of nursing. Must also maintain competencies by participating in a minimum of one educational program per year.

Licensure: Current/Active licensure as an LPN in the state of Rhode Island. Massachusetts LPN license preferred. Active Driver’s license. BLS required.

Experience: One year as a community health nurse in a certified agency with experience and knowledge of JCAHO/Medicare documentation

Working Conditions, Physical Environment and/or Safety Requirements: Requirement to lift 50 lbs. and assist a 200-250 lb. patient, using proper body mechanics. Must demonstrate knowledge of safety precautions. Involves intermittent sitting, standing, walking, and driving motor vehicle. Involves working with frequent interruptions and multiple telephone communications. Requirement to conduct self in tactful, diplomatic manner. Meets all safety requirement associated with the facility. Required to use eyes, hands, fingers with skill in administering therapy and treatments and completing all necessary paperwork. Must follow all Infection Control Policies and Universal Precautions. Regular Staff are required to work the hours for which hired including weekend, holiday, and on-call coverage as established by the Home Health Care Department.
