(Hybrid) Licensed Clinical Social Worker

Psych360, now part of Mindcare, is seeking a hybrid (mix of telehealth and in-person) Licensed Clinical Social Worker (LCSW) to join our team. This role’s primary function is to provide superb counseling services for our client’s residents (primarily nursing homes and assisted-living communities).

Compensation: $65,000 - $70,000 + Bonus Opportunities

Position Summary:

• Design and implement treatment programs
• Provide information regarding community-based organizations and social services
• Evaluate patients willingness to change
• Council/ Educate family members by advising on the most appropriate/helpful roles they can fulfill in the treatment process.
• Consult with other Mental Health professionals
• Complete discharge plans
• Provide brief psychotherapy, crisis intervention and/or personal counseling utilizing individual, conjoint and group treatment modalities

Requirements:

• Past counseling experience
• Licensed Clinical Social Worker (LCSW - State of Pennsylvania)
• Familiarity with the geriatric population and long-care settings is a plus
• Create a climate of patient-centered care to include confidentiality, empathy, respect, and collaboration
• Candidates must have excellent skills in diagnostic assessment and counseling and desire to work in a fast-paced team environment
• A strong work ethic, attention to detail, a willingness to learn, and excellent empathy
• Develop individualized Treatment Plans based on information collected during initial screenings/ evaluations and the ongoing assessment of needs, strengths, preferences, and goals
