Job Summary: The JOB PLUS Lead Family Support Specialist is responsible for supervising and guiding the Job Plus Case Management Team (Family Support Specialist, Job Developer, Workforce Specialist) and ensuring quality controls and accountability throughout the case management process, including during assessment, in the making of referrals to appropriate community services and resources, and in the provision of goal-oriented case management supports that promote housing stability, economic mobility, mental and physical health, and quality of life for East St. Louis children and families.
Responsibilities:
Overseeing implementation of the HAESLJPP's Implementation Stable & Thriving case management program
Supporting and supervising the case management team and the Community Coaches
Leading case coordination meetings with the team to review complex cases and ensure families are receiving needed supportive services and are making progress toward their goals.
Coordinating with the Project Manager and service provider partners to continually assess macro level ESTL needs and progress using continuous data analysis
Monitoring data being recorded in LEARN for timeliness, quality and completeness Preparing reports for the Project Manager that quantify resident progress, challenges and successes
Analyzing data and coordinating strategy development with the case management team to resolve problems and ensure the project is meeting established performance measures Carrying a caseload of families receiving services from the project, fulfilling all duties assigned to the position of Case Manager
QUALIFICATIONS
Education and/or experience required:
Bachelor's Degree in Social Work, Sociology, Psychology or other relevant field. A minimum of three years of work experience as a case manager, service connector, or other relevant direct practice with low-income households, experience with health services a plus.
Commitment to strengths-based and family-focused service planning; ability to understand program philosophies and place-based approach
Ability and commitment to handle privileged information in a professional and confidential manner
Ability to function as a team player in both internal and external relationships
Possession of a valid driver's license and availability of automobile for work use
Strong verbal and written communication skills
Must be proficient in Microsoft Outlook, Word and Excel
Skills and/or competencies required:
Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
Strong organizational, planning and time management skills.
A strong sense of and respect for confidentiality involving both participants and fellow employees.
Must excel in a fast-paced environment
Ability to work in a variety of settings with culturally-diverse persons and communities with the ability to be culturally sensitive and appropriate.
The ability to work independently and with initiative, combined with skills for thriving in a team environment to achieve shared goals.
Computer proficiency in MS Office Suite (Advanced MS Excel), other commonly used software.
