The Integrated Behavioral Health clinician is embedded within the primary care teams, which includes, but is not limited to, primary care providers, outpatient behavioral health clinicians, nurse care managers, community health workers, psychiatric nurse practitioners, and the IBH TEAM to assess patients with mental health and/or substance use disorder, of co-occurring disorders. Clinicians provide brief treatment services within the primary care setting. IBH clinicians work under the direct supervision of the IBH Director, to expand current efforts and implement new and innovative initiatives to support evidenced based integrated behavioral healthcare services, utilizing the SBIRT model, to provide mental health and substance use disorder, including opioid use disorder, intervention, treatment and prevention to the health centers primary care patients.
Responsibilities
- Demonstrating a thorough understanding of Clinical Policies and Procedures for Integrated Behavioral Health, as measured by thoroughness and accuracy of diagnostic formulations brought for review to the multidisciplinary team; and through quarterly Peer Review.
- Triages both scheduled and immediate referrals from the primary care teams, and other primary care support staff, as appropriate, for brief intervention and treatment, outpatient behavioral health, specialty services or to more intensive mental health and/or substance use disorder providers.
- Assisting the Integrated Behavioral Health and Primary Care Teams in the Screening and Brief Intervention, Referral and Treatment (SBIRT) for all patients, through all life cycles, to identify those patients in need of a “warm hand-off” for mental health disorders, such as anxiety and depression, substance use disorder, including opioid use disorder, and for social determinants of health.
- Obtaining complete history and full conceptualization of the patient’s problem within the time limited session(s), as measured by the quality of the written assessment and the timely case presentation to the multidisciplinary team.
- Demonstrating a thorough knowledge of effective treatment protocols for brief solution-focused interventions and necessary cultural competencies for treating children and adults with mental health / substance abuse issues, as well as, chronic disease lifestyle management.
Qualifications
Candidates must have a Master or Doctorate in social work from an accredited college or university. Licensed Clinical or Independent Clinical Social Worker in the State of Rhode Island (LCSW, LICSW). Previous experience or internship in a medical setting which supports a Primary Care/Behavioral Health Integration Model preferred.
